This study is part of the needs assessment of the prevention program Ba-da-lho-ca, aimed at women using harm reduction (HR) services in Barcelona. The objective was to determine the care priorities in the female population from a gender perspective. The methodology used was qualitative, non-experimental, cross-sectional, and descriptive. The
participants were professionals from HR services and women users (cisgender and transgender) of these services. The instruments implemented for data collection, which took place in February-April 2018, were the field diary, the semi-structured interview, and the focus group. The results suggest that: a) HR Model lacks a gender perspective approach
and indicated prevention strategies, b) the lack of differentiated spaces for women in the care services, due to their androcentric design, impacts their adherence, c) professionals have a background in the field and knowledge about the gender perspective, but its application is reduced to crises and d) the group of women highlights the shortcomings of the care network to meet their needs for participation in health, in the community and their
occupations in their life project. Despite its limitations, the present study contributes to the understanding of the low representation and adherence of women to HR services and provides a basis to design
prevention programs that include the feminist perspective, the training of professionals, and the dignification of women as individuals with full rights through community models. It is suggested that the Ba-da-lho-ca Program be implemented to determine the effectiveness of preventive strategies in the key of the sorority
