Abstract

Background During the stay of a premature newborn in the Neonatal Intensive Care Unit (NICU), is exposed to multiple therapeutic or diagnostic procedures, such as capillary heel-lance blood sampling, which causes pain, discomfort and

consequently, physiological alterations. Nowadays there are many non- pharmacological analgesia measures that can avoid neonate’s malaise and

where the nurse has an important role to play. However, scientific evidence affirms due to a lack of knowledge of the collective, many preterm neonates do not receive the necessary analgesia during the procedure, causing hemodynamic destabilisation among other effects. A correct handling of neonatal pain will provide a better development and health status for early infants in NICU.

Objective Evaluate the non-pharmacological measures applied by nursing staff and its effectiveness in the pain management of premature infant in heel-lance blood sampling

Methodology Quantitative, observational, descriptive and cross-sectional study that would develop during the month of January until November 2021. The study sample is not probabilistic and convenient of the nurses from the Neonatal Intensive Care Unit of the Doctor Josep Trueta University Hospital in Girona. A data collection grid will collect variables related to personal nursing data, the technique it performs, the measures it applies and newborn personal data. To assess the level of pain of the newborn before and after the heel-lance, will be used the PIPP assessment scale. 9 order to draw conclusions with the aim of improving the assisting practice of the nursing team in dealing with preterm infants’ pain

Introducció Durant l’estada d’un nounat prematur a la Unitat de Cures Intensives Neonatals és sotmès a múltiples procediments terapèutics o diagnòstics, com és la punció del taló per l’obtenció de mostra sanguínia capil·lar, que li provoquen dolor, malestar i com a conseqüència, alteracions fisiològiques. Avui en dia existeixen mesures d’analgèsia no farmacològiques que poden evitar el malestar del nadó i on la infermera té un important paper a desenvolupar. Malgrat això, l’evidència científica afirma que per falta de formació o coneixement del col·lectiu, molts nounats preterme no reben l’analgèsia necessària durant el procediment provocant-li desestabilització hemodinàmica entre altres efectes. El correcte maneig del dolor neonatal oferirà un millor desenvolupament i estat de salut dels nadons prematurs ingressats a la UCIN.

Objectiu Avaluar les mesures no farmacològiques aplicades pel personal d’infermeria en el maneig del dolor en el nounat prematur davant la punció de taló i la seva efectivitat.

Metodologia Estudi quantitatiu, observacional, descriptiu i transversal que es desenvoluparà durant el mes de gener fins novembre de l’any 2021. La mostra d’estudi serà no probabilística de conveniència de les infermeres de la Unitat de Cures Intensives Neonatals de l’Hospital Universitari Doctor Josep Trueta de Girona. Mitjançant una graella de recollida de dades es recolliran les variables relacionades amb la infermera participant, la tècnica que realitza, les mesures que aplica i les dades del nadó. Per valorar el grau de dolor del nounat abans i després de la punció del taló s’emprarà l’escala de valoració PIPP. Finalment, per elaborar l’anàlisi estadística de les variables i estudiar si són estadísticament significatives s’utilitzarà el programa estadístic informàtic SPSS®.
