Síndrome d'abstinència en cures intensives pediàtriques: incidència i factors de risc: projecte de recerca

Critically ill paediatric patients frequently receive prolonged analgesia and sedation to provide pain relief and facilitate intensive care therapies. Although opioids and benzodiazepines have many established benefits, prolonged administration may induce drug tolerance and physiological dependency. Abrupt discontinuation or too rapid tapering of these medications in physically dependent infants may result in iatrogenic withdrawal syndrome (IWS). Despite the fact that IWS has been observed for decades in infant and children receiving intensive care, it has been only recently that instruments for assessing withdrawal symptoms in Paediatric Critical Care Unit (PICU) patients have been sufficiently validated. Objective: The main purpose of this study is to estimate the incidence of withdrawal syndrome after prolonged infusion of opioids and benzodiazepines. Secondary objectives are to develop a Catalan version of the Sophia Observation withdrawal Symptoms score (SOS), to describe the frequency of IWS signs and symptoms and to study whether IWS management parameters such as medication doses or duration of therapy might be considered risk factors for IWS development. Methods: an observational, descriptive and prospective study will be performed in a Level 2 Hospital PICU, in infant who had received continuous opioid and/or benzodiazepine infusion for at least five days. To assess IWS signs and symptoms, the SOS scale will be used as tool assessment. Descriptive, bivariate and multivariate statistical analysis will be performed using SPSS. As far as we know, this would be the first study in Spain to determine the incidence of IWS in a PICU using a validated tool. The outcomes of the study might result in better strategies for the management of IWS, and basal data to be used in further studies ​
