The pediatric patient usually lives the hospitalization as a traumatic experience, where pain and distress are common in all hospital areas or medical procedurals. In the last years, complementary therapies such a laughter therapy, performed by medical clowns, have proven the effectivity to improve the physical and psychological well-being of hospitalized child. Objectives: To know if the laughter therapy performed by hospital clowns is effective in reducing pain and distress in the pediatric patient. Methods: It has been made a literature review about the last 10 years on the databases of Medline Pubmed, Cuiden Plus, Google Scholar and Cochrane Plus. Results: 102 scientific publications were found and, after applying the inclusion and exclusion criteria, 16 articles have been accepted. Of these, 10 articles have been selected for this present review. Conclusions: Laughter therapy performed by hospital clowns can be effective in reducing distress in any hospital scope, but there is not sufficient scientific evidence to prove that this intervention can reduce the perception of pain in pediatric patients
