Código sepsis: el rol clave del enfermero de urgencias: proyecto de investigación

Abstract

Introducción: La sepsis es una respuesta sistémica inflamatoria causada por una infección. Es una enfermedad comunitaria con un alto indice de morbilidad y mortalidad, y su incidencia aumenta cada año. Debido a su rápida y progresiva evolución, requiere una identificación precoz para abordarla y tomar medidas terapéuticas inmediatas. Su presentación no suele ser bien definida, por lo que es imprescindible la acción de un equipo multidisciplinar para su detección. Alrededor de un 66% de los pacientes sépticos ingresan al sistema de salud por medio del servicio de urgencias. Se han establecido diversos protocolos y guías para dirigir su tratamiento, como la Early Goal Directed Therapy o los paquetes de recomendaciones de la Surviving Sepsis Campaign. Actualmente se han desarrollado en algunos servicios de urgencias, estrategias en formato código para su manejo. El personal de enfermería juega un rol muy importante, ya que está presente en el momento de identificación del paciente séptico en triaje y de la aplicación de las intervenciones recomendadas por la bibliografía. Objetivo: Evaluar el cumplimiento de las intervenciones de enfermería en el manejo de la sepsis y sepsis grave en adultos, realizadas en el tiempo estipulado por la bibliografía, en el servicio de urgencias y emergencias del Hospital Comarcal de La Selva después de la implementación del código sepsis. Metodología: Se realizará un estudio prospectivo, observacional, de carácter descriptivo, y transversal, donde la muestra de estudio es el personal de enfermería del servicio de urgencias del Hospital Comarcal de La Selva. Se analizarán las variables de forma cuantitativa. Se conseguirán los objetivos del estudio utilizando un cuestionario validado, basado en la evidencia científica. Se analizarán los datos obtenidos en el paquete estadístico SPSS v.22.0.0. No se requiere de la aprobación del comité de ética ya que los participantes no son usuarios y la participación es voluntaria

Introduction: Sepsis is the systemic inflammatory response caused by an infection. It is a community disease with a high rate of morbidity and mortality, and its incidence is increasing every year. Due to its rapid and progressive evolution, it requires an early identification to address it and take immediate therapeutical measures. Its presentation is often not well defined, so it requires the action of a multidisciplinary team for detection. Around an 66% of septic patients ingress to the health system through the emergency department. Different protocols and guidelines have been established to guide its treatment, such as the “Early Goal Directed Therapy” or bundles from the “Surviving Sepsis Campaign”. Recently it has been developed in some emergency services, strategies for handling as a code format. Nursing staff plays a very important role, being that it is present at the time of identification of septic patients in triage and in the moment of the implementation of the interventions recommended by the literature. Objective: To evaluate the performance of nursing interventions in the management of sepsis and severe sepsis in adults, conducted in the time stipulated by the literature, in the emergency department and emergency Hospital Comarcal de La Selva after the implementation of the code sepsis. Methodology: A cross-sectional, observational, prospective and descriptive study will be performed, where the study sample are the nurse professionals in the emergency department of “Hospital Comarcal de La Selva”. The variables will be analyzed quantitatively. The objectives of the study will be achieved using a validated questionnaire based on scientific evidence. The obtained data will be analyzed using the SPSS v.22.0.0. software. This study does not require the approval of the ethics committee because participants are users and participation is voluntary
